Membership Application

All members are required to provide the following:

 

  1. Membership application (fill out online application below or download the application and email it to admin@capulc.ca.
  2. Signed Code of Ethics (download a copy).

After filling in the online application, please send the signed code of ethics (all membership levels) by email: admin@capulc.ca or by mail to complete the application process.  You are able to pay your invoice online or mail your payment along with a copy of your invoice to:

Note: Membership applications may take up to 30 days to process. You will receive acknowledgment of membership by email.
